New Student Enrollment Information

Welcome To Moriah Central School

Please fill out all information on the forms listed below and make an appointment to meet with the High School Principal, Mrs. Kathy H. Carr. An appointment can be made by calling (518) 546-3301 ext. 601.  Please bring all necessary paperwork with you to your meeting or there may be a delay in the time it takes for your son/daughter to start school.

Residency Requirements
At least one custodial parent/guardian must live in the district for your child to attend Moriah Central School.  If there are legal custodial arrangements please include a copy of the court order with the new student packet.  Any special circumstances will have to be approved by the Moriah Central School Administration.

Medical Examination Required
After meeting with the principal, please schedule an appointment at the Moriah Family Health Center at (518) 942-7123 for your child to receive a school physical.  There is no charge to you for this service.  Please have the Medical Examination Form filled out by the physician.

Click here to download the Registration Form - Please indicate on this form if your child is currently receiving any Special Education services.  We will work with that department to setup your child’s education plan.
